We are happy to process your insurance claims as a courtesy to you on your behalf.
To do so we require a credit card to be kept on file as well as a portion of each service to be paid at the time of your appointment.
Regular Routine Services – 30% Down payment.
Major Services- 60% Down payment.
All-on-4’s & Implants are not covered by insurance and require full payment prior to treatment.
Insurance policies vary, therefore we can only estimate your coverage in good faith. We cannot guarantee coverage due to the complexities of each insurance contract, however we can send out pre-determinations on your behalf so that you can know exactly what is covered. It is your responsibility to understand your plan, we are not the benefit administrator, however if you have any questions, our courteous staff is always available to answer them as best we can.
